Complex Care Panel Guidance

The individual has either a number of conditions or a high level of risk which means that the nature of their support is complex, intensive and may be unpredictable, requiring hospital admission or intervention from services. Cases should be submitted to this panel if they may require joint funding by Health, Adult Care and Children's services.

  1. Panel application should only be completed following a Multi Disciplinary Team Meeting regarding individual cases. If appropriate, negotiations must be carried out by the procurement teams in the department of the lead worker, regarding the cost of the recommended package prior to any submission to Panel;

  2. The embedded documents should be used for all applications:
  3. Panel applications should also be supported by individual professional assessments from either Health or Social Care;
  4. Lead practitioners from Health and Social Care should complete the application, make recommendations and attend Panel;
  5. Lead officer to send to Eve Moroney at the Complex Care Team, 10 days prior to Panel, application and supporting assessments;
  6. Applications will then be sent to Panel members 1 weeks before the date of the Panel;
  7. It is envisaged that the case discussions at Panel are likely to have one of three outcomes:
    • Agree;
    • Refuse;
    • Defer for more information.
  8. Letter to be sent from Panel to the client/ patient by the Complex Care Team, copies also sent to the lead practitioner for patients' file.

    In all cases applications must:
    • Be supported by relevant assessments;
    • Have costings and be fully completed;
    • Procurement must be involved prior to Panel.
